TEHSEEKERS PTE. LTD.

This Medical Declaration and Health Acknowledgement is required for participation in any activity organised, facilitated, or promoted by TEHSEEKERS PTE. LTD., hereinafter referred to as TehSeekers. This declaration forms part of the Terms and Conditions governing participation.

1. Declaration of Fitness

I declare that I am physically and mentally fit to participate in outdoor, trekking, and adventure activities which may involve physical exertion, uneven or unstable terrain, altitude exposure, adverse weather, remote locations, and limited or delayed access to medical facilities.

2. Disclosure of Medical Information

I confirm that I have fully and truthfully disclosed all relevant medical information, including but not limited to pre existing medical conditions, injuries, illnesses, allergies, medications, physical limitations, mental health conditions, or any other factor that may affect my ability to participate safely.

I acknowledge that failure to disclose or misrepresentation of medical information may increase my risk of injury or harm and may affect the assistance that can be provided.

3. Personal Responsibility and No Medical Advice

I understand and acknowledge that TehSeekers does not provide medical advice, diagnosis, treatment, or health assessments. Responsibility for determining my fitness and suitability to participate rests entirely with me.

I confirm that I have sought independent medical advice where necessary and that I voluntarily choose to participate at my own risk.

4. Medication and Self Management

I confirm that I will carry all required personal medication and understand its correct usage. I accept full responsibility for managing my medical needs during participation, including timely administration of medication and disclosure of emergency instructions where relevant.

5. Emergency and First Aid Consent

I authorise TehSeekers, its representatives, guides, or partners to administer basic first aid and to arrange emergency medical assistance, evacuation, or treatment if deemed necessary for my safety. I understand and agree that all costs associated with such assistance or treatment shall be my sole responsibility.

6. Limited Medical Access Acknowledgement

I acknowledge that many activities may take place in remote or challenging environments where immediate medical assistance may not be available and evacuation may be delayed due to terrain, weather, or logistical constraints.

7. Right to Restrict or Discontinue Participation

I acknowledge that TehSeekers reserves the right, at its sole discretion, to refuse, restrict, or discontinue my participation if my condition, conduct, or health is assessed as unsafe or unsuitable, without refund or compensation.

8. Accuracy and Ongoing Disclosure

I confirm that all information provided by me is true, complete, and accurate. I agree to promptly inform TehSeekers of any change in my medical condition prior to or during the activity.

I accept full responsibility for my health, wellbeing, and any consequences arising from inaccurate, incomplete, or withheld information.

9. Acknowledgement

I confirm that I have read, understood, and voluntarily accepted this Medical Declaration and Health Acknowledgement and that I understand it forms part of the binding agreement governing my participation.
